Transition Pack

Fully editable, symbolised resources to help prepare learners for changes throughout their education

The symbolised Transition Pack is designed to support children at each stage of their school journey. It helps parents and educators prepare learners for changes such as moving from nursery to school, between classes or key stages and into secondary school or post 16 education.

A wide range of editable activities can be used at home or in the classroom to enable parents and practitioners to personalise preparation for individual learners and also support whole class teaching.

The pack includes clear, supportive Stories to Help which can help reduce anxiety and build understanding around change. Photobooks and visual prompts provide practical, consistent support before, during and after transitions.

Together, these resources promote confidence, predictability and positive transition experiences for learners.
